Thank you for applying to the University of the Nations! May you know the Lord’s grace as you seek His direction. In order for us to process your application, we must receive all of the following completed forms. If a question does not apply to you, write N/A in the blank. Husbands and wives as students must complete separate applications.

o 1. Entry Application Form (P-001). This form must be filled out for any initial course you wish to apply for at the UofN in Ukraine. Note: All dates are requested in an international format: day, month, year.

o 2. Application Fee. A nonrefundable Application Fee of $US 40 for singles or $US 60 for a couple is to be forwarded with the application. Your application cannot be processed without it.

o 3. Personal History. Please prayerfully and concisely answer the following questions on a separate sheet of paper (printed or typed) and attach it to your P-001 form. Your answers will be significant in the application process.

a. Please describe your conversion experience and present relationship with the Lord.

b. What areas of your character are you presently seeking God to further develop and improve?

c. Please describe your spiritual and/or ministry goals, including missionary service goals.

d. Please describe your relationship with your local church, i.e., areas of ministry, service, leadership experience, gifts and abilities.

e. Please describe your business, professional, missions or other significant experiences.

f. How would you describe your relationship with your family?

g. How would your family feel about your plans to apply for training with Youth With a Mission, (U of N)?

h. Do you believe you could live under pioneer situations, including different food and culture, dormitory housing or small quarters for families?

I What is your specific commitment to missions; short and long term?

o 4. Reference Form (P-002). After you sign and enter your course and date, give one form to your pastor, employer or teacher, and a friend. If you have taken a YWAM course previously, one of your references must be from your most recent school leader. Ask them to complete the form and mail it directly to Ukraine. Please list their names and addresses on a separate sheet of paper. We must receive at least 2 of these reference forms (pastor’s form must be one) before we can process your application.

o 5. Health Form (P-003). Please complete this form and return it directly to Ukraine. These forms must also be submitted for your children. Note: this form is attached to the application form (P-001) when you download it.

TESTS & IMMUNIZATIONS: YWAM Ukraine requires that all students and children must be tested for TB, and show evidence of a negative skin test or clear chest X-ray. Please fill in this information on the P-003 or on a separate report. Documentation must clearly indicate the test performed, the results, and the examination facility where the test was performed. You are also required to take an AIDS test and bring the results with you. Please be up to date on Tetanus and Hepatitis shots also.

o 6. If you intend to enroll in a UofN degree program, please send us the following:

a. Transcripts. You must request that a transcript of your High/Secondary School and / or College / University / Seminary record to be sent to U of N. (A transcript or letter of good standing is required even if no credits were earned.)

b. Proficiency/Aptitude Test for International Students from countries where English is NOT the native language. TOEFL (Test of English as a Foreign Language) or equivalent. Scores obtained in aptitude or language proficiency tests must be submitted with your application.

Important notes:

-All students are encourage to apply early, and generally no later than two months prior to the start of the school.

-You must obtain or apply for a passport before coming! This normally takes 2 months for a passport and an additional 1 month for a Ukrainian visa- obtained through a consulate in your country.
